By returning this completed form, I agree to abide by the Clubs code of conduct which I have read.

I give permission for the Club to contact me from time to time with information and activities that may be of interest to me.

Once you have completed and submitted the form feel free to join us for two complementary pool swims on Tuesday evenings at 9pm. Just let the Coach know.

If accepted for membership I agree to pay the annual subscription of £25 which will be invoiced via email.
